- Also include a copy of your teaching certificate if you have already received it.

- Criminal History Act 34 Form - Please read the directions on the form carefully. You should send the form to Harrisburg for processing as soon as possible. It takes at least two weeks to process. If you already have an Act 34 form within the last 12 months, it is not necessary to complete another one. Submit a copy with your application. If you are contacted for an interview, you must bring the original form with you. Applicants who have not resided in the Commonwealth of Pennsylvania for the last two years must also submit a copy of the Federal Criminal History Record Information report from the Federal Bureau of Investigation. Please contact our office if you need the FBI record request. You will need to report to a State Police barracks for fingerprinting.
- Child Abuse Act 151 Form - You must provide us with a copy of the Pennsylvania Child Abuse History Clearance from the Department of Public Welfare. This clearance can be completed online by following the link provided on this page. Once you are redirected to the Compass website you will need to select CREATE INDIVIDUAL ACCOUNT so that you are able to apply for this clearance. If you have not created an account before, you will need to create one. An email will be sent with your Keystone ID and a password. When applying for this clearance please make sure to check the "School Employment" option. The results will be available on the Compass website. If you do not have online access a form can be obtained from the Childline and Abuse Registry, Department of Public Welfare, P.O. Box 8170, Harrisburg, PA 17105-8170 (phone 717-783-6211).

- FBI Background Check Act 114- To get started, you will need to enter Service Code 1KG6XN for EMPLOYMENT. Please read the directions carefully. You can make an appointment online at one of the fingerprint centers (the closest are in Altoona and Huntingdon). (Act 114 went into effect April 2007). At your appointment, you will receive a receipt that your fingerprint information is being processed. If hired, the report must be submitted within 90 days of hiring.
* When registering on-line, an applicant must use the appropriate agency specific Service Code to ensure they are processed for the correct agency and/or applicant type. Using the correct service code ensures the background check is submitted for the correct purpose. Fingerprint requests processed through any other agency or purpose cannot be accepted and are not transferrable. If an applicant enters the wrong code by mistake, the incorrect applicant type will appear at the top of the screen. The applicant should select the “Back to Home” button and begin the process again, by reentering the correct Service Code. If the applicant proceeds with the process under the incorrect code, the pre-enrollment and/or results cannot be transferred to another state agency and the applicant will have to start the process over and pay for the background check again. Volunteers- Criminal History Act 34 - Volunteers will need to click "New Record Check (Volunteers Only)." There is no charge for requests made for the purpose of volunteering. The volunteer record check can be made via the PATCH website or by mail using form SP4-164A, "REQUEST FOR CRIMINAL RECORD CHECK, VOLUNTEER ONLY." Please read the directions on the form carefully.
- Child Abuse Act 151 - You must provide us with a copy of the Pennsylvania Child Abuse History Clearance from the Department of Public Welfare. If you have not created an account before, you will need to create one. An email will be sent with your Keystone ID and a password. When requesting clearances, be sure to select "Volunteer Having Contact with Children: Applying for the purpose of volunteering as an adult for an unpaid position as a volunteer with a child-care service, a school or a program, activity or service, as a person responsible for the child's welfare or having direct volunteer contact with children." According to DPW, "As a volunteer applicant you are permitted to receive one certification free of charge every 57 months. In order to submit a volunteer application without a payment, you are required to affirm that you have not already received a paper or electronic volunteer certification free of charge within the previous 57 months."
- FBI Fingerprinting Waiver - Volunteers who are not required to obtain the FBI certification because they are applying for an unpaid position and have been a continuous resident of Pennsylvania for the past 10 years must swear or affirm in writing that they are not disqualified from service based upon a conviction of an offense under §6344.

If a volunteer has not been a resident of Pennsylvania for the previous 10 years, but obtained their FBI certification at any time since establishing residency, they must provide a copy of the certification to the person responsible for the selection of volunteers and they are not required to obtain any additional FBI certifications.
If a volunteer has not been a resident of Pennsylvania for the previous 10 years, and has not obtained their FBI certification, they must obtain that certification.

If you are hired by the District, it will be necessary to have a Health Form completed by your physician prior to your first day of work. At this point, please be prepared to have an exam, if requested. If you already have a current completed health record done within the last three months, you may submit it with your application. Your application will not be considered active until all the necessary forms are completed and on file. If you are not selected for an interview, your application will remain active for one year.
